One Dayton nursing facility sold, another to transition operations

A pair of nursing facilities in Dayton are in transition.

The 323 Forest Ave. property of Forest View Care and Rehabilitation Center in Dayton was just sold for nearly $1.7 million, property records show.

Also in Dayton, The Cottages of Clayton will close its skilled nursing facility at 2212 N. Main St., but no jobs are expected to be lost.

The facility’s executive director notified the Ohio Department of Job and Family Services of the plan in a letter dated Sept. 21.

“The closing is anticipated to occur 60 days from the writing of this letter,” Justin Carwile, the facility’s executive director, said in the letter to the state. “This action is expected to be temporary, as the facility intends to license its SNF (skilled nursing facility) building as an assisted living facility immediately following the closure of SNF operations.”

Meanwhile, at 323 Forest Ave., the 37,372-square-foot building sold this week along with land parcels totalling about 3.5 acres.

The property sold Wednesday to Forestview Propco LLC from Sabra Health Vcare, according to Montgomery County property records. The site is close to Grandview Medical Center, at 405 W. Grand Ave.

The site’s legal business name is Harborside of Dayton Limited Partnership, according to Medicare records.

A web site says the facility is operating as Gem City Nursing & Rehab.

Forestview Propco is a domestic limited liability company formed in July, with a Chagrin Falls, Ohio mailing address.
